For your first visit, please provide Insurance information, doctor’s prescription, and any personal specified details pertaining to your needs. This may include:
Private Insurance forms
Medicare card / secondary insurance card
Current Medicaid card spend-down information
H.M.O. authorization from primary physician or medical group
Worker’s Compensation - bring addresses and phone numbers of contact person to guarantee payment for device prescribed.
